1. Health Protection is an insurance of the insurance companies Sanitas, S.A. de Seguros and BanSabadell Seguros Generales, S.A. de Seguros y Reaseguros, as co-insurers, each underwriting 50%. Sanitas Sociedad Anónima de Seguros, tax ID A-28037042 and registered offices at C/ Ribera del Loira, 52, 28042 Madrid, Spain, is registered in the Mercantile Register of Madrid and the Insurance Companies Register of the General Directorate of Insurance and Pension Funds under code C-320.

BanSabadell Seguros Generales, S.A. of Insurance and Reinsurance with Tax ID no. A-64194590 and its registered offices at C/ Isabel Colbrand, 22, 28050 Madrid, Spain, registered with the Mercantile Register of Madrid and the Insurance Companies Register of the General Directorate of Insurance and Pension Funds under code C-0767.

Insurance brokered by BanSabadell Mediación, Operador de Banca-Seguros Vinculado del Grupo Banco Sabadell, S.A., and marketed in the network of Banco de Sabadell, S.A., entity with which it has signed a distribution network services contract. 2. Delivery of your medicines from the pharmacy to your home within a maximum of 3 hours (6 deliveries per insured person per year).

3. Two home-based clinical analysis services are included per insured person per year. If using a laboratory not contracted for the provision of the Blood Testing Service at the Insured Party's home, the following maximum reimbursement limits shall apply: €60 for travel and €200 for the home blood testing service. This service will be part of the policy, free of charge, throughout the effective term thereof unless informed otherwise by Sanitas.
